Our client a catholic faith based institution in Nairobi is seeking to recruit a religious, devoted, focused and competent professional in the position of a social worker as outlined below.
Vacant Position: Social Worker Liaison: All In-Charge
Job Summary: The key objectives for this position are to work towards the vision and mission and according the strategic plan of the institute, do follow-ups- of all clients’ and help in administrative work
Key Duties and Responsibilities
Admitting of clients (complete admission forms) according to laid policies. Home visits/follow-ups & writing of case summary of the visited studentsDocument case study/case historyComplete reports for (monthly, half year and yearly) requirements.Maintain updated records.Refer clients to other appropriate social services/other organizations Help in organizing for the preparation of beds for new clients and provide them with personal items as needed.Enhance contact between client and natural family to help in the reconciliation process.Give basic health care and home care instructions to the accommodated clients.Journey with clients through sharing, listening, guiding them as need arises.Medical follow up, accompany client to health care facility, i.e. Hospital, visiting/seeing to those admitted.Up-date client’s admission form by attaching the required documents and taking their photos to attach to their forms.Carrying out interviews and intakes (first year students) Admission of new students (filling & filing forms) when needed. Attending to parents Keeping in order the required files i.e., admission, home visits, Case follow ups.Manage donations in-kind and ensure they are distributed to the students end term (mostly clothes)Schedule home visits/follow-ups, keep updated documentation.Assist to organize and plan outings, activities, guest speakers, etc. as needed.Maintain accurate records and documentation needed for reports.Maintain library books, borrowing and return logs.Manage program communication with other staff, students, and staff in charge, parents and agencies. (internal and external communication)Participate in program planning, meetings, workshops as needed.Attend parent meetings, & events or other related activities as needed.Work with the whole team to provide the beneficiaries with the best services.Keeping in order the required files i.e., admission, home visits, etc.Provide counselling and resettlement of clients.
